Please do not use this facility to request changes to your medications.
The surgery offers a secure online scheme that is fully integrated with our practice system.
In order to you use the system for re-ordering repeat prescriptions or booking appointments you must first register by completing the application form. This can be printed from the link below, along with a copy of the practice guidelines.
The service is free to our registered patients over 16 years of age and is available day and night, up to 365 days a year.
Requesting your online account details
If you wish to use the online facility we ask that you read a copy of our Practice Information Leaflet and then you must complete and sign an application form. These are available in reception or you can download them by clicking the links below.
Return your completed form to reception or post it to your usual surgery. We will then set up your account and create a letter containing all your account details.
For security reasons you must collect this letter from the practice in person as we cannot give out account details by phone, post or email.
If you have poor mobility and are unable to get to the surgery, you may nominate a relative or carer to collect the details on your behalf (at your own risk) by writing their details on the application form.
Please allow at least five working days for us to set up your account before coming to collect your letter, and bring some photo identification with you (such as a passport, student card or driving licence) as we will need it to verify your identity.
Creating your account
After you have obtained the letter containing your account details click on the link at the top of this page. A new window will open up (entitled Patient Access) from which you need to select the option to register.
It is important that you enter your details exactly as they appear in the letter we have provided.
When you create your account you will be asked to choose a password. Please do not reveal this to anyone. If at any time you believe that someone else may have learned your password or gained access to your account you should contact us as soon as possible.
